How to add pdfs for your index

I'm sure this info is available, but I can't find it.
I want to include The World's Greatest Fake Book. I own it legally and have loaded its index onto my iPad Air. I can scan the book's pages, but have no idea what to do next -- how to name them and get them to connect to the index, etc.

This tutorial covers some critical aspects about this in an interactive fashion.

The biggest thing to make note of is that when you have an index with not PDF connected to it you get the visual cues such as grayed out titles or if your view displays thumbnails, you have a thumbnail that says "PDF Not Present". When you select a song title from an index without a PDF attached to it you get a pop up message that tells you EXACTLY what name the PDF should be to associate it with that index.
