Indexing Bulk Upload - Rise Up Singing -- multiple songs on one page

In the past, I have used the bulk upload successfully in the Content Manager (and in fact have an Excel template to help me). But now I have a problem figuring out how to code a page that has two or three songs on a page.

Using the template:
"[song title]",[page number],[number of pages],"[composer last name, composer first name]"

I started with
"America",1,1,"Simon,Paul"
"America (West Side Story)",1,0,"Bernstein,Leonard"
"Battle Of New Orleans",1,0,"Driftwood,Jimmy"
"America The Beautiful",1,0,"Ward,Samuel A.&Bates,Katharine Lee"
"Camptown Races",1,0,"Foster,Stephen"
"Be Prepared",2,1,"Lehrer,Tom"

and get this error: "incorrect data format in line 2" (no matter what song I put in). What is the correct format?

Having number of pages as zero doesn't make any sense since it will at least be one page, the number or pages should be 1 or greater never zero.
