If you have a PDF linked to an index and that PDF is missing pages, follow these steps:
1. Unlink the PDF from the index by pressing and holding that item in the Book List View. Index Linking 101
2. Review the PDF from first page to last to figure out what pages are missing: Checking A PDF Page By Page
