Unable to add my book to my.igigbook.com

I tried following the instructions here: How To Create Your Own Book Index For Your Custom Book viewtopic.php?f=22&t=7 and I fill out the fields "Book Index Name", "Songs", "Book Index Code" , "Visible" and "PDF Password" then I click "Add New" but no book seems to be created and I get no error message. What am I missing?

Apparently, I was using too long an index. I created a shorter index name and it worked and when I tried to change it to my original index name, I get an error message. It would be nice to see the error when I adding the book in the first place.
