I am using the Android version 5.4

I can't get the index to sync to the physical pages. With no offset, as examples here is what I get when I select the following songs. See table in code tags at the bottom:

Columns:
1. Song Title
2. When the song is selected from the list of songs in the book, this is the page number shown above the chart
3. This the page number that was actually retrieved by the index in the physical book
4. This is the page in the physical book where this song actually appears
5. This is the content that appears in iGigBook when the song is retrieved

As you can see from this handful of examples, there is no offset number that will work for all of them. When I set the offset to work for some songs, it won't work for others. In addition, the page number shown in iGigBook above the chart matches the printed page number on each chart page, but this is not the physical page number in the book.

I would like to find out:

How to get a copy of the PDF file that will match the index
or
Extract the index so I can edit my copy of the PDF file to match the index
or
Any other ideas you've got

I’m not sure what physical book you have but the index we have for the “The Standards Real Book” has the song “After You” as page 1 and is the C version of the book. The index in this book also lists the song “After You” as being on page 1. If your book has the song “After You” on any page other than 1, it is not the book that corresponds to this index. No amount of adjustments will make your book work with this index if it doesn’t meet this requirement.

In the original printed book there are several pages of front matter before the first song After You. The page number in the corner of the printed page for the song is 1, but the song is not on the first physical page. My PDF file is a faithful copy of the print version.

As I said, even if I adjust the offset so that the first song comes up correctly, the offset breaks down later in the book.

If the index is not based on the actual book, what is it based on?

I can edit my file to insert or delete pages to match the index, but there appears to be no way to export the index to make this an easier task. The only way I can figure out how to do this is to sit with my tablet and click each individual song, see what page number comes up, and then match that up with the PDF file pages. That's much more time-consuming than having a ready list of the songs and page numbers from the index.

You must go through each page of your PDF to make sure all of the pages are present and in the correct order. Looking at your PDF it's clear that page 4 is missing. You haven't reviewed your own PDF.

You don't need to export an index. Each page in the PDF is numbered which means you start with the first song which is page 1, "After You" and just tab the bottom of the screen to advance to the next page which will be page 2 until you get to the last page, which is 561, "Yours Is My Heart Alone". Write down on a piece of paper which pages are missing and then you can insert the missing pages.

Looking at your PDF it's clear that page 4 is missing. You haven't reviewed your own PDF.

I have not gone through it page by page, no. However, your helpful comment has led to me to find that the book has many pages that are just photos, and these pages are omitted from my version. I suspect that if I create a new version that fills in blank pages for these photo pages, and omits the front matter, it may sync with the index.
