Your index lists 624, the book print states 'more then 625'. I counted 636 in my paper book.

I ended up making my own index for the book, as I didn't know it was supposed to be in your list already. Feel free to compare my index to yours.... In any case, your index doesn't seem to line up properly with book I have titled 'the ultimate Fakebook' edited by dr. Wong.
